At Joint Effort Physical Therapy, your needs are our highest concern. We understand how important it is for you to be seen in a timely manner, and we accommodate appointments immediately upon request. You may be seen by one of our staff member during our office hours:
Mon: 6:30 am – 7:00 pm
Tues: 6:30 am – 7:00 pm
Wed: 6:30 am – 7:00 pm
Thurs: 6:30 am – 7:00 pm
Fri: 6:30 am – 4:00 pm
For new patients, we ask that you please arrive 15 minutes prior to your appointment to allow enough time to fill out necessary paperwork. Please also take the time to review our Notice of Privacy Practices and Welcome Letter below. Our Welcome Letter addresses many commonly asked questions. Additionally, you can print the intake form below, fill it out, and bring it to your evaluation visit. If this is completed ahead of time, you do not need to arrive early for your first visit.
